1. The petitioner has started an action in the above court requesting that your marriage be dissolved.
2. The petition also requests that the Court grant the following relief: Dispose of property and liabilities. Change the name of the wife to: Valerie Lynn Boulton.
3. You must respond to this summons by serving a copy of your written response on the person signing this summons and by filing the original with the clerk of the court. If you do not serve your written response within 60 days after the date of the first publication of this summons (60 days after the 9th day of January 9, 2008), the court may enter an order of default against you, and the court may, without further notice to you, enter a decree and approve or provided for other relief requested in this summons. In the case of a dissolution of marriage, the court will not enter the final decree until at least 90 days after service and filing. If you serve a notice of appearance on the undersigned person, you are entitled to notice before an order of default or a decree may be entered.
4. Your written response to the summons and petition must be on form WPF DR 01.0300, Response to Petition (Domestic Relations). In the matter of the estate of:
DOROTHY SULLIVAN,
Deceased.
THE PERSONAL REPRESENTATIVE NAMED BELOW has been appointed as personal representative of this estate. Any person having a claim against the decedent must, before the time the claim would be barred by any otherwise applicable statute of limitations, present the claim in the manner as provided in RCW 11.40.070 by serving on or mailing to the personal representative or the personal representative's attorney at the address stated below a copy of the claim and filing the original of the claim with the court in which the probate proceedings were commenced. The claim must be presented within the later of: (1) Thirty days after the personal representative served or mailed the notice to the creditor as provided under RCW 11.40.020(1)(c); or (2) four months after the date of first publication of the notice. If the claim is not presented within this time frame, the claim is forever barred, except as otherwise provided in RCW 11.40.051 and RCW 11.40.060. This bar is effective as to claims against both the decedent's probate and non-probate assets.
