The event is scheduled from 1:00 PM to 4:00 PM at 6400 Sylvan Way SW, Seattle, WA 98126, specifically at the Neighborhood House - High Point location
These vital services are available at no cost to everyone, regardless of insurance status.
Patients will be seen on a first come, first served basis, as the event operates on a walk-in only model.
Services provided by Care-a-Van and Bird’s Eye Medical include:
• A1C Testing for those 35 years and older.
• Blood Pressure Screening for individuals aged 3 years and above.
• Blood Glucose Screening for those 12 years and older.
• Mental Health Screening for individuals 12 years and older, along with broader Mental Health Services.
• Telehealth Referral services.
• BMI Screening for those 2 years and above.
• Vaccine Education for all ages
• Naloxone (Narcan) Kit Distribution
• General Health and Vaccine Education.
It's important to note that parental consent is required for patients under the age of 18 years old. For insured patients, health insurance information will be obtained.
